Yes. As long as your opponent has either an unsuspended level 4 or level 5 in Digimon in play, you can activate this card's effect.

Does this card have to have both Lv.4 and Lv.5 opponent's Digimon in active state for it to be effective?
No. It is activated even when only one of the opponent's Digimon is in active state, either Lv.4 or Lv.5.
